All patients who are on HRT need to have a review three months after starting and every twelve months thereafter.
Your annual review can be done by completing a questionnaire providing the original prescription was issued by Bron y Garn Surgery and you are happy with and wish to continue with your current therapy.
Please complete this form and submit it at least 2 weeks before your next HRT prescription is due.
After review by a GP you will be issued with a 12-month supply if appropriate.
