Long Term Condition Care

An integrated system of care is offered to patients with long term conditions such as asthma, diabetes and heart disease.

We are now changing the way that we undertake your annual review for your Long-Term Condition(s). We are aiming to complete your annual review during your birth month, this will make it easier for us all to remember and prompt when you are due to be seen.

If you suffer with more than one Long Term Condition, we would like to complete all the relevant reviews during one visit. You may have had your annual review a few months ago but we would like to see you again to re-set your annual reviews to align with your birth month.

Patients are encouraged to set up reminders within their birth month and are welcome to book for their annual review in advance.

You will be invited by text / phone call or a letter to book your appointment.

For a lot of the Long-Term Condition Reviews excluding Asthma & COPD, you will require a blood test and will need to provide a urine sample. If you could bring a urine sample along with you to your appointment, preferably complete on the day. You can pick up urine sample pots from the surgery prior to your appointment.

Dependant on the outcome of results for your initial Annual Review, you may not be required to see a doctor. If it is decided that a doctor’s appointment is required, then the Surgery will contact you to arrange.

If you suffer with Asthma or COPD you will need to bring your inhalers with you to the appointment.
