Any patient registered with the practice between the ages of 16 & 75 who has not attended any consultation or clinic within the previous 3 years may request a consultation. If any patient over the age of 75 has not had a consultation within the previous 12 months, may request a consultation either at the surgery, or if this is not possible for medical reasons, the patients home.
